This is a simple document that you complete; it will instruct your family / survivors, and/or your church or pastor, your wishes concerning immediate plans that you have prepared in advance.

Those How, Why, Where, When, questions concerning the deposition of your remains; and many other important decisions that must be made within 48 hours after a death occurs.

The Declaration of Intent is not a will or a legal document, but a personal message to those you leave behind.

What this means to you is that you can leave written instructions for your family and church, concerning your final plans; rather then the guesswork of what some might think that you probably wanted. 

The real benefit is because you left these final instructions for your family; there is less conflict among family members, and those last memories, concerning those last days, are memories of thoughtful considerations of a loved one.
